Required Forms

Tentative acceptance/slotting into encampment is conditional on receiving all required/applicable forms.  Some of these forms are required by the Civil Air Patrol, some by the State of Maryland for Summer Camps, and one by the Military for orientation flights. Fillable electronic forms have been created (below) for you / your parents/guardians to fill out and electronically sign.  Once you've completed each form, be sure to "Submit" them.  This will automatically send your documents to the Encampment Cadre for review and acceptance. Once submitted you'll get a "Thank You" screen that has a "Download PDF" button in the bottom right of that screen.  Download, save and print the PDF to bring with you to encampment.

The Maryland medical forms below, require a wet/original signature from a physician.  You will need to print the applicable form, fill it out, and ensure that a parent/guardian signs the form before providing it to a physician. An electronic copy of the completed/signed Maryland medical forms can then be submitted using the links below. A hardcopy of the Maryland medical forms with wet/original signatures should be brought to encampment and turned in during in-processing.

Students must also submit electronic copies of their immunization records and health insurance cards. A link to submit that health documentation is also provided below.

NOTE: Senior Members are only required to submit the CAPF 60-81, 160, 161, and DOD Form 1381


CAP Required Forms for Encampment

NOTE: Upon receipt of an online application, a CAPF 60-81 will be sent to the parent/guardian who will sign, along with the student, before the form is finally sent to the squadron commander for signature. Please check your emails for this form and fill out/sign promptly.

Student Health Documentation

An electronic copy of student immunization records and health insurance cards must be submitted to the encampment.
