Required Forms
Per directions listed on the Student Registration Zone for Tri-Wing Encampment (Maryland/Delaware), applicants must download all required forms found in Registration Zone.
Print, complete, include original wet signatures (ink), and save a copy of each document as a PDF. Return to Registration Zone and upload each file to its respective section for verification. Incomplete or missing forms may impact slotting, so please ensure to download the forms early and upload them prior to the deadline.
The format for each file name should begin with your last name, first name, followed by the name of the document.
● FORMAT: LastName_FirstName_DocumentName.pdf
● EXAMPLE: Smith_John_CAPF161.pdf
** Retain all of your original forms to bring to encampment for in-processing.

1. CAPF 161 - Emergency Information Form

2. CAPF 163 - Cadet Over-the-Counter Medication Authorization Form

Complete, including original wet signature (ink). Cadets who will be 18 during encampment may sign the form for themselves

3. TWE Health Disclosure Form

4. Health Insurance Card into Registration Zone

If you do not have health insurance, just upload the PDF intentionally blank

Bring 1 copy of the FRONT and BACK of your Insurance Card to encampment
5. Immunization (Vaccination) Record

Members who do not get vaccinated must upload a statement of vaccine exemption

Bring 1 copy of your Immunization Record to encampment
6. 305 OG Health Statement for Military Flights

Complete, including original wet signature (ink)

Bring original signed form to encampment
7. Over 18 - Mil Flight CAP Cadet Hold Harmless Agreement AND Under 18 - Mil Flight CAP Cadet Hold Harmless Agreement

Complete the form (Over 18 or Under 18) that pertains to the cadet, including original wet signature (ink)

Upload the completed form as well as the blank form as PDF copy to Registration Zone

Bring the original signed form to encampment
NOTE: Both forms are listed as required on Registration Zone. For the version that does not apply, simply upload a blank document to satisfy the requirement (for example: A 13-year-old applicant will complete the Under 18 Form and upload it to Registration Zone, and also upload a blank copy of the Over 18 Form to its respective section).
8. Parental Photo / Name Release Preference: Please fill out this online form
** Retain all of your original forms to bring to encampment for in-processing.
NOTE: Senior Members are only required to submit the TWE Health Form, CAPF 161, 305 OG Health Statement, and the Hold Harmless Agreement.
