Required Forms

Fill out and submit all applicable forms below to be considered for Encampment.

Final acceptance into encampment is conditional on receiving all required forms. Some of these forms are required by the Civil Air Patrol and some are required by the State of Maryland for Summer Camps. Fillable electronic forms have been created (below) for you / your parents/guardians to fill out and electronically sign. Once you've completed each form, be sure to "Submit" them. This will automatically send your documents to Encampment Staff for review and acceptance.

The required Maryland medical forms below, require a wet signature from a physician. Fill out the electronic form, then submit. Once submitted you'll get a "Thank You" screen that has a "Download PDF" button in the bottom right of that screen. Download, save and print the PDF to take to your physician for signature.

● Hard copies of all wet signed forms must be emailed to the encampment staff at the address above, as well as be brought to encampment and turned in during in-processing.
